IMPORTANT HEALTH COVERAGE ACA TAX DOCUMENTS:  The City of Belton has made Form 1095-C available for current and former employees. These forms provide information about your health coverage for the 2025 tax year and may be needed when preparing your federal income tax return. You may request a copy of your Form 1095-C by contacting the Human Resources Department.

Copies may be mailed or picked up in person at City Hall during regular business hours, Monday through Friday, 8:00 a.m. – 5:00 p.m., excluding federal holidays.
